Community Case Management
The mission of PSL Services's Community Case Management is to support the participant's access to the community and resources that will enhance their participation in the community they live and their quality of life.
Representative Payee Services
PSL Services currently provides representative payee services to community case management individuals served by our agency at no cost. A representative payee will receive your monthly benefits and use the money to pay for your current needs to include housing, utilities, food, medical expenses, etc. as well as assist in any paperwork associated with benefits.
If you or your family member needs this type of service, please feel free to discuss this with our agency, during the intake process.
Assessment/planning
A thorough assessment is conducted, working with the participant to identify their specific needs.
Next we develop a detailed plan that identifies the participant's goals and the services that will be provided.
Meet regularly with the participant to evaluate if the services we are providing are meeting the goals that were identified in the plan and make changes to the plan as needed.
Services
Health and Wellness this service is designed to promote, support, and maintain the participants over all health and may include coordination and arrangement of:
medical appointments
mental health services
nutritional and fitness supports
Physical and occupational therapy services
Quality of Life
A category of services that balances freedom of choice and individual life styles with personal responsibility and safety. The focus is always on promoting the participant's competencies that will result in safety and freedom or abuse and neglect.
Services may include:
- Assisting in securing information on services that are available, so decisions are informed choices.
- Assistance in obtaining and coordinating legal resources.
- Assist in the coordination or mediation of problems with housing, employment and other provider of services
- Resolutions that may arise
- Assistance with legal issues and involvement with the police.
Community Membership is a group of services designed to assist and enhance the participant's understanding of and accessing the neighborhood and community in which they live. Examples of this service include:
- Assist participant in accessing community groups, and organizations that reflect their personal interest. E.g. Spiritual, YMCA, classes.
- Identifying and assisting in volunteer opportunities.
- Assist in accessing local support group.
Information and Referral is a group of services designed to ensure that the participant has access to information, which will enhance the quality of their life. Services may include assisting access into:
Assist and coordinate referrals to federal assistance programs such as social security and food stamps.
Identify and help maintain state administered services such as MaineCare, Section 8 rent subsidies, and fuel assistance.
